HURON SHORES FAMILY HEALTH TEAM

The East Algoma agencies seeking to form an Ontario Health Team (OHT), proudly announce they have been selected by the Ministry of Health and Long Term Care to proceed with development.
The East Algoma team with membership that included health and social service sectors and patient representation has been acknowledged for demonstrating a strong history of trusting relationships among partners, integration, shared financial management, and a commitment to improve the patient experience to reduce hallway medicine.
Over the next several months, the East Algoma OHT will be working to ensure readiness to proceed to the full development of a strong rural model. This model strives to provide care close to home as well as establishing partnerships that robustly enables patient flow between the rural and urban health care services.
The East Algoma OHT proposed geographic boundary encompassing nearly 8,750 square kilometers includes the area from Echo Bay, Ont., east to Spanish, Ont. and up to Elliot Lake, Ont.
The establishment of OHTs is designed to promote the collaboration of healthcare agencies in delivering coordinated care and helping patients to better navigate the system.
The East Algoma OHT congratulates the Algoma OHT and the Équipe Santé Sudbury & District OHT for being recognized as ready to proceed to the next steps in the development of their teams. The East Algoma OHT looks forward to working with these teams in the future to provide patients with a seamless experience between transition points of care.
